I certify that the above information is true and that I have read and agree to Daily Transcription Terms of Services. By initialing this document you understand that all clients must have a valid credit card on file with DT. This does not constitute a billing option. You will receive an invoice for your project, which must be settled by project completion. In the event that you default upon invoice payment, you authorize DT to charge your credit card for all completed work done by DT. Please note, your project output will be held until payment has been made. You understand that if you are approved for the pay by check option that payments are due within 30 days of invoice date and are considered past due after 30 days. For pay by check clients, DT reserves the right to charge the credit card on file for all past due invoices in addition to applicable fees. By accepting this agreement you authorize Daily Transcription to charge your credit card $100.00 which will be applied to your total bill. This does not reflect the total amount you may owe, but only to get the project started. Upon completion of your work, you will be charged the remainder of the balance owed or credited if your balance is less than $100. An invoice will be sent via email stating the total for the project. You are responsible for all work submitted.
